We are seeking a skilled RN/LVN to provide nursing care to our Home Health patients.
For Location:
Calexico, El Centro, Brawley, Imperial.
As SN, you will play a vital role in providing quality care to our patients.
This position requires strong clinical and communication skills.
RN Duties:
- Complete an initial assessment of patient and family to determine home care needs.
Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Counsels the patient and family in meeting nursing and related needs.
LVN Duties :
- Provides direct care as defined in State's Name Nurse Practice Act - Implements plan of care initiated by registered nurse.
-Provides accurate and timely documentation consistent with plan of care.

Monday to Friday Weekends as needed Work Location:
In person.
Estimated Salary: $20 to $28 per hour based on qualifications.
